Please fill out a Transcript Request Form (PDF). Please note: the form must be signed. You may bring it to Dunwoody in person or mail the form in to Student Records. Transcript requests cannot be taken over the phone due to the need for signature and a $5 fee.

Note: If you cannot view PDF form you can usually work around the problem by downloading the file to your computer before opening it. Here is how you do that:

1) Right click on the website link to the “Transcript Request Form (PDF).” Do NOT open it at this time.
2)Choose “Save Target As…” or “Save Link As….” from the pull-down menu. The “Save As” window will appear.
3)In the “Save In” box at the top, select a location to save the file on your computer that will be easy for you to remember. Your desktop or my documents are the two most common locations. Click on “Save”.
4) The PDF file will now download to your computer. After the download process is finished, navigate to the location where you saved the file and double click on it to open it. You should be able to open and print the file from this location without any problem.
5) Drop-off, e-mail, fax or mail completed form to the Registrar’s office.

It may take up to 5 days to process your transcript once the request and payment is received. If you would like to pick it up in person, please make a note of this on the form. This process applies to requests for both official and unofficial transcripts. Your mailed/written request for transcripts must include:

  • Your full name;
  • Dates you attended;
  • Whether attended Dunwoody or NEI;
  • Your Social Security number;
  • The complete mailing address of where you want your transcripts sent; and
  • A check for $5 payable to Dunwoody College of Technology. NOTE: There is no charge for current students for either official or unofficial transcripts.

Dunwoody cannot provide copies of your transcripts from other colleges attended. Such documents must be requested from the other institutions in question.

Verification of Status for Employers
We are able to verify standard “directory” information as directed by FERPA; so long as a student has not given written notification refusing such disclosure. There is no fee for this service. Directory information includes:

  1. Student Name
  2. Dates of attendance
  3. Course of study/major
  4. Degrees earned
  5. FT/PT status
  6. Honor List status

*Additional information may be available with written consent of the student/graduate – please provide signed release form and specific information authorized for release.

To request verification of information please provide us with adequate identifying information and fax your written request to (612) 234-4346. Be sure to include:

  • Date of graduation;
  • Dates of attendance;
  • Student’s Social Security number; and
  • Whether or not the student attended Dunwoody College of Technology or NEI College of Technology (the 2 colleges merged in 2003).

We will return the completed request for information to you within 5 days.
